The five core components of Medication Management Therapy include:
- Medication Therapy Review (MTR): A detailed, comprehensive assessment of all medications (prescription, over-the-counter, supplements) to identify potential issues, interactions, duplications, or gaps in treatment.
- Personal Medication Record (PMR): A clear, up-to-date document listing every medication you’re taking, including names, dosages, instructions, purposes, and prescribing providers—empowering you to stay organized and informed.
- Medication-Related Action Plan (MAP): A customized, step-by-step plan outlining how to manage your medications, resolve identified concerns, improve adherence, and address any barriers to success.
