Discovery Transitions is a high-quality drug and alcohol outpatient treatment facility located in Southern California. We believe in treating addiction as a three-fold illness: mind, body, and spirit.
We offer outpatient treatment programs for individuals seeking structure and a program designed to address each client individually.
Our primary goal is to encourage our clients to maintain their commitment to recovery while receiving the necessary treatment to live a productive life, free from drug and alcohol dependence.​
Our Approach
At Discovery Transitions Outpatient, we prioritize a holistic, individualized approach to treatment. An on-site psychiatrist is available to address any medication-related questions or needs that clients may have.
Our clinical team is trained in a variety of therapeutic modalities, allowing them to identify and implement the most effective path for each person.
Therapists partner closely with clients to support goal achievement, sustain long-term sobriety, and promote overall mental health and well-being.
Upon admission, clients receive an initial treatment plan developed from their intake assessment by their assigned therapist.
Within the first few days, the multidisciplinary treatment team collaborates with the client to create a comprehensive, personalized treatment plan.
This team reviews and updates each plan weekly throughout active treatment to ensure ongoing relevance and progress.
Clients typically meet with the psychiatrist within their first week—and regularly thereafter—for medication management as needed.
They also connect with their primary therapist once or twice weekly to address their current needs and set realistic, achievable goals per session.
Treatment duration is fully customized to each individual’s circumstances, maximizing the likelihood of lasting success and recovery.
