SFBT: Solution Focused Brief Therapy

Solution-Focused Brief Therapy (SFBT), also known as Solution-Focused Therapy (SFT), is a forward-looking, collaborative, and goal-oriented therapeutic approach that prioritizes the present and future over an in-depth exploration of the past. Rather than dwelling on the origins or details of problems, SFBT centers on defining clear goals, identifying solutions, and building on the client’s existing strengths and resources. Clients are encouraged to experiment with practical skills in their everyday lives, systematically overcoming barriers by amplifying what already works.

Therapists employ carefully crafted questions—such as the miracle question (“Suppose a miracle happened overnight and your problem disappeared—how would you notice?”), scaling questions (“On a scale of 0–10, where are you now, and what would move you one step higher?”), and exception-finding questions (“When has this problem been less intense or absent? What was different then?”)—to guide clients toward envisioning success, testing new behaviors, and leveraging past successes to address future challenges.

Approach SFBT emerged as an alternative to traditional problem-focused psychotherapies, emphasizing partnership with the client, who is viewed as the expert on their own life. The therapist collaborates to co-construct realistic, actionable solutions that lead to meaningful change in a relatively short time frame. This respectful, efficient method is grounded in the belief that solutions often lie in the “exceptions”—those moments when the problem is absent or less dominant—and that small, positive shifts can create momentum for larger transformations.

Principles

  • If it’s not broken, don’t fix it: Avoid unnecessary intervention; build on what is already effective.
  • Look for exceptions: Identify and amplify times when the problem is absent or manageable to replicate those conditions.
  • Ask questions: Use purposeful, future-oriented questions as the main tool to evoke change and insight.
  • Focus on the future: Direct attention toward desired outcomes and solutions rather than past causes.
  • Compliments: Affirm and reinforce the client’s strengths, efforts, and successes to build confidence and motivation.
  • Encourage what works: Support continuation and expansion of helpful behaviors.
  • Change is constant: Highlight and utilize even small, ongoing improvements.
  • Solutions may not relate directly to the problem: Effective solutions can sometimes emerge unexpectedly and need not mirror the problem’s structure.

Applications SFBT is versatile and has been successfully used in individual therapy, couples counseling, family therapy, schools, workplaces, hospitals, prisons, community settings, and more. It is particularly helpful for issues like depression, anxiety, substance use disorders, behavioral challenges, relationship difficulties, parenting concerns, and life transitions. It shines when clients have some awareness of potential solutions or strengths they can build upon, and when rapid, practical progress is desired.

Evidence of Effectiveness SFBT is considered an evidence-based practice, supported by numerous randomized controlled trials, systematic reviews, and meta-analyses. Recent research (including umbrella reviews and meta-analyses up to 2024–2025) shows significant positive outcomes across diverse populations, issues, and cultural contexts, with effect sizes ranging from moderate to large in many cases. It demonstrates benefits comparable to (and sometimes exceeding) longer therapies like CBT in certain areas, often in fewer sessions (typically 5–8). There is no evidence of harm, and it is effective for psychosocial functioning, well-being, mental health symptoms, addiction reduction, and more.

Limitations While efficient and empowering, critics note that SFBT’s brief, solution-oriented focus may not provide sufficient space for deep empathy or exploration of underlying trauma, unconscious conflicts, or complex origins of behavior. For clients who need to process past experiences or severe conditions (e.g., schizophrenia or major depression), SFBT alone may be insufficient and is often best combined with other approaches. Some argue it risks oversimplification, and more high-quality, long-term research is ongoing to refine its applications.
