Discovery Transitions Outpatient began with one man’s promise and lives on in every person we welcome through our doors.

Our Story

It started with someone who understood.

Discovery Transitions wasn’t founded in a boardroom. It was founded by a man who had walked the hard road of addiction himself — someone who knew firsthand what it feels like to struggle, to start over, and to search for people who truly understand.

When he found his own recovery, he made a promise: to build the kind of program he wished had existed for him. Not just treatment, but a true community. A place where people in recovery could learn the practical skills to rebuild their lives, and — just as importantly — find a support network that would stand beside them long after the program ended.

That vision became Discovery Transitions Outpatient. From day one, it was different, because it was built by someone who had lived it. Every group session, every life skill taught, every late-night phone call answered came from a simple belief: lasting recovery grows out of lasting connection.

Our founder has since passed away, but what he built did not end with him. It couldn’t — because he never built it for himself. He built it for every person who would ever walk through our doors looking for a second chance. His story is woven into everything we do, and honoring it is the heart of our work.

The founder’s chair may be empty, but his mission fills every room.

Tracy Marie Flinn

Executive Director

Tracy’s story is woven into the story of Discovery Transitions itself. For nine years, she worked side by side with our founder — learning not just how he ran this program, but why: the second chances, the open door, the unshakeable belief that no one is ever too far gone. When he passed, there was no one better prepared to carry his vision forward.

Today, as Executive Director, Tracy leads with the same heart she learned from him. This work holds a special place in her heart — it always has — and it shows in every decision she makes. Under her leadership, the standards our founder set aren’t just maintained. They’re honored.

Olga Skarlato

Clinical Director · LMFT

As a Licensed Marriage and Family Therapist, Olga understands something essential about recovery: it never happens in a vacuum. Addiction touches families, relationships, and every corner of a person’s life — and healing has to reach just as far. As Clinical Director, she shapes the care behind every part of our program, making sure it’s grounded, thoughtful, and built around the whole person.

Olga holds our clinical work to the highest standard, but what clients remember is her warmth. She creates a space where it’s safe to be honest, safe to struggle, and safe to start again — because in her experience, that’s where real healing begins.

Shari Colato

Program Director · Certified Counselor

Shari has dedicated more than a decade to behavioral health, and it shows in the way she leads: with steadiness, warmth, and a deep respect for every person’s story. As Program Director, she carries our founder’s vision forward every single day — making sure that what happens inside our walls is never just treatment, but genuine connection.

As a certified counselor, Shari brings both professional expertise and real heart to her work. Clients know her as someone who listens first, meets them exactly where they are, and never gives up on anyone. If you call Discovery Transitions, there’s a good chance Shari’s team will be the warm welcome on the other end.

Monica Parr

Admissions Director · CADC

For most people, Monica is the first voice they hear at Discovery Transitions — and that’s no accident. As our Admissions Director and a Certified Alcohol and Drug Counselor, she brings real counseling experience to the very first conversation. When you call, you’re not talking to a salesperson. You’re talking to someone who understands what you’re going through and knows how to help.

Monica makes the hardest phone call of someone’s life feel a little less scary. She listens without judgment, answers every question honestly, and walks each person and family through their next steps with patience and care. Reaching out takes courage — Monica makes sure it’s met with warmth.

Aldo Leyva

Counselor · CADC

As a Certified Alcohol and Drug Counselor, Aldo works with clients where recovery actually happens: in the day-to-day. He brings patience, honesty, and a calm presence to every conversation, helping people untangle the hard stuff and build something better in its place.

Clients trust Aldo because he keeps it real — no lectures, no judgment, just genuine support from someone who believes in their ability to change. He’s one more reason that at Discovery Transitions, you’re never walking alone.
