As a transition program, we are ideal for anyone who has just completed an in-patient program and is ready to step down to a less intensive but highly structured continuation of treatment. Discovery Transitions are equally effective and available for those whose substance abuse problems are severe enough to require help but do not rise to the level of requiring hospitalization and residential treatment.
